MP4 video output for iPad is just a movie file. It doesn't retain interactivity. However, there is a (somewhat convoluted) way to publish Captivate content for iPad that DOES retain the ability to have interactivity and quizzing: http://blog.easelsolutions.com/2011/05/publish-captivate-5-to-the-iphoneipad/

None of the video formats available in Captivate 5.5 are compatible with Google Docs.

In fact, there isn't any significant difference between the different presets.

What changed between Captivate 5.0 to 5.5? I used to be able to publish videos that worked in Google Docs.
